INSURANCE
We are out-of-network with commercial insurances. Please check with your insurance company to determine out-of-network coverage for outpatient physical therapy. Dry needling and Wellness appointments are not covered by insurance. Payment will be due at the time of service. We will then submit claims to your insurance company on your behalf or provide you with a superbill for reimbursement.
MEDICARE
We are in-network with Medicare. Claims will be submitted to your primary and secondary insurances. Per your insurance contract, you are responsible for any copay/coinsurance. For Medicare to cover services, a prescription from your physician is required.
NO
INSURANCE
Save an average of 30% with HSA/FSA. HSA/FSA accounts use pre-tax money to pay for medical expenses. These funds may be used for all services. Please contact us to inquire about discounted packages.
